Appointment availability chooser

Does anyone know of some type of plugin or script that can do this?
What I need is for clients to indicate which days they are free to take
appointments.
A good example of what I need can be found at
http://www.makeurmove.co.uk/expressRoute/let?product_id=1 at the bottom of
the page.
The website I need it for currently uses jQuery with PHP. A solution in
either' or languages would be appreciated.
It doesn't need to be a week by week basis, a 1 week view is preferred.
